Flag or unflag a XC connection with data issues

Flags or unflags a connection as having data issues (e.g. the XC lead found a wrong port or cabinet while reviewing the request). Internal users only: business users get 403 even if they hold the permission.

Flag (has_issues: true): sets has_issues, issues_flagged_by and issues_flagged_at.

Unflag (has_issues: false): clears the three fields. No comment is required.

Comment (optional): explains where the issue is. It is saved as an XC comment linked to the connection (xc_connection_id), PUBLIC by default or PRIVATE with comment_type. It is created in the same transaction as the flag change.

Idempotent: flagging an already flagged connection (or unflagging one that is not flagged) returns 200 with the current state and does not overwrite issues_flagged_at. A comment, if sent, is still created.

Response: everything the UI needs to update without refetching: the full updated connection (same shape as GET /v1/xc/connections/{id}), the created comment (same shape as the comments endpoints, or null), and the request's updated connections_with_issues count.

Notifications: follow the XC comment rules. A PUBLIC comment, or a new flag sent without a comment, emails the same recipients as a PUBLIC XC comment (global XC admins and the request's requester) with a link to the request's comments tab. A PRIVATE comment emails no one.

Status block: while any connection of a request is flagged, PATCH /v1/xc/requests/{id}/status only accepts a move to Revision (409 REQUEST_HAS_CONNECTION_ISSUES otherwise).

has_issues cannot be set through the generic connection POST/PATCH endpoints.

IC / Extended IC: PATCH /v1/ic/connections/{id}/issues behaves the same for icc_/eicc_ connections and requires ic-connection-update. The path ID accepts the xcc_, icc_ and eicc_ prefixes, and each route only finds connections of its own module (404 otherwise).
