Maintainer:Not configured

Saves a user's answers to the COVID questionnaire for a visit, scores them, and emails the result. A risky result bans the user for 15 days; a safe one is valid for 3 days.

Public and unchecked: anyone can submit answers for any user ID, which also overwrites that user's birthday, height, weight and gender with empty values.

Public: no session token is required.
